If you have or think you may have attended these places you must follow the NSW Health directives to self-isolate and get tested immediately.
You MUST NOT attend our practice if you have ANY Covid-like symptoms, or if you have attended any of the places listed on the dates listed; until you have had a negative COVID test result and you are free of symptoms.
